Social care is any help that you need to live your life as comfortably and independently as possible, because of age, illness or disability.

Tameside Adult Social Care supports people over 18 years old who live in Tameside.

Care and support can look different for everyone.

Help could be short term to get you back on your feet and feeling more independent, for example support from our reablement team or signposting to other services which could benefit you. 

If needed, following an assessment we can also work with you to plan longer term support. This could be help with washing, dressing, getting out the house or connecting with local community groups. We can look at what services, equipment or adaptations could assist you to be independent as possible, so you can live well in the place you call home.

Our Vision

We all want to live in the place we call home with the people and things that we love, in communities where we look out for one another, doing things that matter to us.

Our vision see’s people living well at home, where they are supported to be a part of their local communities to live long, healthy and fulfilled lives. 

This vision describes the social care future for Tameside and sets our intention for how we want to develop our services, so everybody can be supported to achieve their goals and aspirations.

Our Plan

Our Adult Social Care Strategy guides our vision, by outlining our key priorities and explaining how we will deliver them.

Living Well at Home Practice Framework

Our practice framework explains how our organisation works, what guides our approach, and how we support frontline teams. It also helps us understand the experiences of people receiving our support, making sure they stay at the heart of what we do. 

Having a clear framework helps us to deliver consistent, high-quality support by setting out what is expected and how we work together. 

This not a strict rulebook; instead, it is a flexible guide that respects professional judgement and adapts to the diverse needs of a community like Tameside, because what works for one person might not work for another. 

This framework is here to help our teams focus on what really matters: building strong, meaningful relationships with adults and their carers to make a real difference in their lives.

Supporting Carers

Looking after a loved one often comes at great personal cost. We know that many sacrifice their own time, energy and money to care for someone.  

Working with our Carers Centre we have a dedicated Carers Strategy, outlining how we are supporting carers to live well in the place they call home.
